FIVE YEAR ANESTHESIA REEVALUATIONS
If you perform any type of sedation in your office, you need to be reevaluated every five years. Please contact an active ISOMS member to complete this reevaluation for you.
If you practice in more than one office location, please also complete the Satellite Office Affidavit.
If you do not perform any type of sedation in your office, you will only need to complete the OAE Exemption form.
PROTOCOL FOR LATE FIVE YEAR ANESTHESIA REEVALUATION
- If OAE is going to be late or is late, inform Laney Dezelan with a scheduled date (30 day extension will be given)
- If OAE is not performed within that 30 days, a certified letter will be sent (15 day extension given)
- If OAE is still not performed, call from ISOMS Board Member (15 day extension given) with notice given that you will be dropped from membership if your evaluation has not be performed within that 15 days.
- If OAE is still not performed, you will be dropped from membership on that date. You will be sent a certified letter informing you that have been dropped from membership with further instructions.
